YOUR RESERVATION FORM
Your Reservation Form must be completed in its entirety. You must submit all forms before your Reservation can be processed. We will mail information in regard to the Payment Program with your invoice for initial payment.
To make a reservation you can either:

Fill out the Reservation form an submit by email before June 22, 2023 for this rate;

mail it with your deposit before June 22, 2023 for this rate;

Single rates are 200% of the published fare
(Includes Port fees & Taxes)
Deposit (non refundable/transferable): $50.00 per person due now, to secure your spot.
THE BELOW PAYMENT SCHEDULE IS THE SUGGGEST SCHEDULE
First Payment due Now ~ Or ~ on or before June 22, 2023
Second Payment a total of $180.00 due on or before July 1, 2023
FINAL PAYMENT OF REMAINING BALANCE IS DUE ON OR BEFORE AUGUST 15, 2023

CANCELLATION: If you miss your monthly payment, our reservation is subject to cancellation. Again, please note that all monies paid are NON-REFUNDABLE including if a hotel package was involved. If you must cancel for any reason, written documentation will be required. If your cabin mate cancels, you are still responsible for the full cabin and any hotel costs.

IMPORTANT NOTES - PLEASE READ:

If you are reserving for two people in the same cabin who reside at different
addresses, and want to have your confirmations sent to each person, then you
must fill out separate forms for each traveler. Put the appropriate deposit amount
in each traveler's separate reservation.
Remember to include your cabin mate’s name in the “Cabin Mate” field, even if
you are not making a deposit for them.

If making reservations for two or more at the same address, only one form is
required and the deposit should reflect the number of passengers (ie: 2 people =
$100, 3 people = $150). -
REMEMBER, ALL GUEST MUST COMPLETE ALL FORMS IN ORDER TO PROCESS THE RESERVATION

We at Elegant Liz Travels strongly recommend that you purchase Trip Cancellation
Insurance to protect your travel investment. The cruise line offers insurance. The
cruise line quote (Trip Cancellation is $49.00 per person) or you can go to: www.
travelguard.com for a quote. If you are not purchasing the Trip Cancellation
Insurance, a Travel Insurance Acceptance/waiver form must be completed and
forward to your travel agent.

Persons with pre-existing health conditions must purchase insurance at time of
booking or within 15 days of booking.

Passports are HIGHLY RECOMMENDED...But at this time are not needed. PLEASE
BE AWARE: without a passport or passport card, you will not be permitted to
depart from most islands by air even in case of an emergency.
